Moving Into Long-Term Care

Application Process

All long-term care applications are submitted through your local Home & Community Care Support Services office. Applicants must be over 18, possess a valid Ontario Health Card, and have care needs that can be met in a long-term care community.  

When You Accept an Offer

Home & Community Care Support Services will recommend that you contact your chosen long-term care community to make arrangements for a move-in day. You will receive information from the community on what you should bring with you, and what to expect in your first days of settling in. The community team will review all of your needs with you and welcome you to share any important information about your wishes and requirements.

Transition & Getting Settled

Once you’ve been assessed and officially offered a room, you are welcome to move-in on the pre-arranged date. We provide a bed, bedside table, dresser, and closet or bureau. We encourage you to make the space your own – consider bringing small personal items, a TV, photographs and pictures for your wall, etc. – we want your new space to feel like home.  

Spousal Reunification Process

Spousal Reunification is a special provision that allows your spouse or partner to come and live with you in your long-term care home. If this is something you are interested in, please speak to your Home & Community Care Support Services Coordinator or to our Resident & Family Experience Coordinator for more information and to find out if you may be eligible.   

Here is a quick step-by-step guide to the process:

 

  • Complete an application for long-term care through your local Home & Community Care Support Services team (visit https://www.healthcareathome.ca/ or call 310-2222 (no area code required). They will work with you to assess your needs and determine your eligibility.
  • Decide what type of accommodation you would like to apply for – basic, semi-private, or private.
  • Home & Community Care Support Services will ask you to provide them with a list of up to five long-term care communities based on your preference. Visit https://www.healthcareathome.ca/long-term-care/selecting-a-home/ to learn more about choosing the right location for you.
  • If you can, tour your chosen long-term care communities before making your final decision.
  • Home & Community Care Support Services will send applications to your chosen list of long-term care communities.
  • Once a community accepts an application, Home & Community Care Support Services will notify you that you have been added to that community’s wait list. Please note that wait list times vary depending on where you are located – your Home & Community Care Support Services Coordinator can help you understand the timelines for your area.
  • You will be notified by Home & Community Care Support Services as soon as accommodation becomes available – which could be at any of the long-term care communities on your list – and you will have 24 hours to make a decision to accept or decline an offer, with up to 5 days from the date of your acceptance to move-in.
  • If you decline the offer, you will need to reapply for long-term care again, and will be placed back on the waiting list.
